NHS menopause care can be excellent, and we would never suggest otherwise — but ten minutes is rarely enough for a decade of overlapping symptoms, and specialist waiting lists are long. You do not need hot flushes to be perimenopausal.
Symptoms fluctuate, overlap and often start years before periods stop. For many women the first signs are anxiety, poor sleep or brain fog — which is exactly why they get attributed to something else.
Vasomotor symptoms
Hot flushes, night sweats, palpitations and sudden changes in body temperature — the classic signs, but by no means universal.
Brain fog and memory
Poor concentration, word-finding difficulty, forgetfulness and a marked drop in confidence at work.
Mood and mental health
Low mood, irritability, anxiety, tearfulness and reduced resilience to everyday stress.
Sleep disturbance
Difficulty falling asleep, repeated night waking, early waking and unrefreshing sleep, with or without night sweats.
Genitourinary symptoms
Vaginal dryness, discomfort during sex, itching, urinary urgency and recurrent urinary tract infections.
Joints, bones and body
Joint aches, muscle pain, morning stiffness, frozen shoulder and changes in bone density.
Cycle changes, PMS and PMDD
Heavier, lighter, closer or skipped periods, worsening premenstrual symptoms and increasingly unpredictable cycles.
Libido and intimacy
Reduced desire, difficulty with arousal and loss of confidence with intimacy.
And what else it might be
Thyroid disease, iron deficiency, ADHD, sleep disorders and burnout all mimic menopause. A proper assessment looks for these too, rather than assuming.
A menopause assessment is more than a symptom checklist.
Every initial consultation covers the following as standard, in a single unhurried appointment.
The full picture, properly taken. A complete menopause and medical history, validated symptom scoring, and examination where clinically indicated. Breast cancer risk, cardiovascular risk and bone health are all considered before any recommendation is made — not afterwards.
Wider wellbeing. Sleep, mood, sexual wellbeing, urinary symptoms, weight, nutrition and exercise are all part of the conversation, because they are all part of how you actually feel.
A plan, not a deferral. Your appointment ends with an individual care plan, which may include practical recommendations, further investigations, follow-up or referral where appropriate.
Joined up with your GP. With your consent, a summary letter goes to your GP so your records stay accurate and your ongoing care stays coordinated.
Reviewed, then reviewed again. We see you again at around four weeks to review your symptoms, discuss how your care plan is working and agree any appropriate next steps.
