What is the BPA Form 115a?

Form 115a is the official British Skydiving (formerly BPA) medical certificate, signed by a registered doctor to confirm you're medically fit to skydive.

It's a separate document to Form 115b — the self-declaration you fill in yourself. If everything on 115b is "no" and no other criteria apply, you may not need 115a at all. If anything triggers, that's where we come in.

Our GMC-registered GPs review your declaration, run the required clinical checks, and complete and stamp the 115a in the appointment. You leave the clinic with everything your drop zone needs.

Who Needs Form 115a

You'll need a GP-signed medical if…

Most jumpers fall into one of these categories. If you're not sure, book in and we'll work through your 115b with you — bring everything you've got and we'll confirm what you need on the day.

You ticked "yes" on Form 115b

Any "yes" answer on the self-declaration triggers a GP medical, even if the condition feels minor or historical.

You're over the age threshold

Drop zones often require a GP-signed 115a from age 50 or 60+, depending on activity (tandem, AFF, licence holder).

Recent surgery, fracture or admission

Anything in the last 5 years — operations, broken bones, hospital stays — needs reviewing before you jump again.

You take regular medication

For chronic conditions like blood pressure, diabetes, asthma, thyroid or mental health — most are fine, but need GP review.

Heart, lung, neuro or mental health history

Past or present — well-controlled conditions are usually cleared, but the GP needs to formally sign you off.

Returning to the sport

After a long break, an instructor course, a tandem renewal or a B-licence application — your drop zone may require fresh paperwork.

How It Works

Most jumpers walk in with their 115b half-completed and walk out with the lot done. Here's exactly how the visit goes.

Pick a slot using the booking widget below and pay £59 securely. Bring photo ID, your part-completed Form 115b, your NHS app so we can review your medical data, and a list of any medications.

15-minute consultation

You'll see one of our GMC-registered GPs at our Standish clinic. They'll review your history, do the required checks, and discuss anything flagged.

Walk out signed off

If you're cleared, your 115a is completed, signed and stamped before you leave — with both a printed and a digital copy ready to send to your drop zone.

How quickly can I get an appointment?
We typically have same-week slots available. Use the booking widget above to see live availability — if you need something sooner than the next visible slot, ring the clinic and we'll see what we can fit in.
What do I need to bring on the day?
Photo ID (passport or driving licence), your completed (or part-completed) Form 115b, a list of any current medications, and details of any conditions, surgeries or hospital admissions in the last 5 years. Bring glasses or hearing aids if you wear them.
I have a heart condition / take regular medication — can I still get cleared?
In most cases, yes. The GP's job is to assess whether your condition is well-controlled and compatible with skydiving — not to refuse on principle. Bring any recent specialist letters or test results if you have them. If we can't sign on the day, we'll explain exactly what's needed (a specialist letter, a recent ECG, etc.) so you can come back.
Is the form accepted at all UK drop zones?
Yes. Form 115a is the standard British Skydiving medical certificate and is recognised at every BS-affiliated drop zone in the UK. Our GPs are GMC-registered and the form is signed and stamped to BS standard.
How long is my 115a valid for?
It depends on your age and individual circumstances. For most jumpers it lasts up to 5 years. Shorter validity periods apply if you're older or have a flagged condition — the signing GP will write the exact validity on your form.
Do you sign 115a for AFF, B Licence, instructor and tandem requirements?
Yes — the same Form 115a covers AFF students, licence holders, instructor renewals, tandem passengers and category jumpers. Bring any specific paperwork your drop zone or instructor has given you and we'll complete it alongside.
What if I'm not cleared on the day?
It's rare. If you do need additional information (a specialist letter, a recent test result, etc.), we'll explain exactly what's required and what to bring next time. Most jumpers come back fully cleared once any flagged item is addressed.
