Understanding Mast Cell Activation Syndrome (MCAS): Diagnosis & Treatment Options

Mast Cell Activation Syndrome (MCAS) is a complex condition caused by excessive mast cell mediator release, leading to symptoms such as hives, gastrointestinal issues, flushing, fatigue, and sometimes anaphylaxis.
At Ready Health in Standish, our GP appointments are designed to listen, assess, and act. We take the time to understand your symptoms and, where MCAS is suspected, provide referral to one of only three UK specialists able to investigate and diagnose the condition.
With NHS pathways often limited, our private GP service offers a timely, reliable route to diagnosis and management of MCAS.
Following a confirmed diagnosis, we can also provide ongoing care, support, and access to medications to help manage the condition long term.
Diagnosis of Mast Cell Activation Syndrome
Diagnosing MCAS can be a complex process as symptoms vary widely between individuals and often overlap with other conditions such as histamine intolerance or allergies. At Ready Health, we work closely with our UK specialists partnerships and can arrange appropriate investigations, which may include:
Tryptase levels
24-hour urinary histamine or prostaglandin metabolites
Detailed symptom assessment and medical history
We provide ongoing care and support, taking the time to listen and ensuring a personalised approach for every patient throughout their diagnostic and treatment journey.
Treatment Options for MCAS
Managing MCAS typically involves a combination of trigger avoidance and medication to stabilise mast cells or block their effects. At Ready Health, we offer access to a range of treatments, including:
1. Sodium Cromoglicate (Nalcrom Capsules)
This mast cell stabiliser is particularly effective for gastrointestinal symptoms and is commonly prescribed as part of MCAS treatment. However, Nalcrom (Sodium Cromoglicate) may not be readily available on the NHS in all areas due to prescribing restrictions or supply issues.
✅ We can source and supply Nalcrom capsules privately. If you’re having trouble obtaining this vital medication, contact us and we’ll help.
2. Antihistamines
Both H1-antihistamines (such as cetirizine, loratadine, or fexofenadine) and H2-antihistamines (like ranitidine or famotidine) are commonly used to block histamine and reduce symptoms like itching, flushing, and gastrointestinal upset. These are often taken regularly in MCAS, not just during acute reactions.
3. Leukotriene Receptor Antagonists
Medications such as montelukast may be used to reduce inflammation related to mast cell mediator release.
4. Adrenaline Auto-Injectors (EpiPen)
Anyone diagnosed with MCAS should always carry an adrenaline auto-injector (e.g. EpiPen, Jext, Emerade) due to the risk of sudden severe reactions, including anaphylaxis. We can prescribe and provide guidance on safe use of auto-injectors, and ensure you are prepared in case of emergency.
